Your “Final Evaluation on Student Progress” on page 5 of the Form I-983 signed by you and your former employer
A new Form I-983 (page 1-5) signed by you and your new employer

Once your SEVIS record has been updated, OISSS will mail your I-20 by regular US Postal Service to your address within 5 business days. You may choose other options below to receive your I-20.
