Immigration Check-in Form

In accordance with U.S. immigration regulations, all F-1 and J-1 students are required to check-in with the Office of International Student and Scholar Services (OISSS) upon their arrival at Eastern Kentucky University (EKU). To check-in, complete this online Immigration Check-in Form and submit your passport, visa and Form I-20 or DS-2019. You may also turn in your I-94 record. Download and Print I-94.
