Practicum and Internship Overview


This guide is designed to help students through the Practicum/Internship (P/I) application process. Students must apply each semester. It is important to follow all instructions.

NOTE: If you know a potential practicum/internship site that is not listed in our database, please contact the Practicum/Internship Coordinator. We are always looking for new sites to serve the needs of our students.

Practicum and Internship Application Deadlines


March 14, 2025
All SUMMER 2025 clinical mental health counseling practicum and internship applications are due.
All FALL 2025 school counseling practicum and internship applications are due.

April 21, 2025
All FALL 2025 clinical mental health counseling practicum and internship applications are due.
Practicum or Internship applications can be accessed through Tevera. If you do not have a Tevera account, please contact LeAnna Hensley.

Practicum and Internship Application Requirements


Please visit the Site Placement tab on Tevera for the most current list of requirements:

  • Application
  • Proof of Professional Liability Insurance (Not your personal medical/auto insurance)
  • Review of Clinical Handbook and Signed Acknowledgement
  • Clinical Placement Waiver
  • Place of Employment Verification (submit only if doing P/I at place of employment)
  • Site and Site Supervisor Information
  • Orientation Verification
  • Background Check (practicum only)

After the Close of the Application Window:

  • Student Orientation: Required of all students applying for Practicum and First Semester Internship
  • Student Summer 2025 Orientation Date: Friday, April 25 at 5:30pm (Zoom link to be sent prior to the orientation)
  • Student Fall 2025 Orientation Date: August 1 at 5:30pm (Zoom link to be sent prior to the orientation)
  • Background Checks for Practicum Students: Additional information will be sent after the close of the application window.
  • Telehealth Training for Practicum Students: Additional information will be sent after the close of the application window.
  • CRN for Course Registration: Will be sent approximately three weeks after the close of the application window to all students with completed P/I applications.
  • GPA Submission: Will be sent via Tevera.

Experience Documentation (to be completed DURING P/I) (NOTICE: It is each student’s responsibility to keep copies of all documentation for personal/professional records required for licensure.) The following documents should be completed via Tevera.

  • Counselor-in-Training Disclosure/Informed Consent form (Do NOT upload to Tevera)
  • Permission to Record form (Do NOT upload to Tevera)
  • Supervision Contract (Practicum/Internship Agreement)
  • Weekly Clinical Hours Log
  • Activity and Summary Hours Log form
  • Midterm PCPE Evaluation form (to be completed by Site Supervisor via Tevera)
  • Final PCPE Evaluation form (to be completed by Site Supervisor and University Supervisor via Tevera)

Site Evaluation (to be completed at the END of the P/I experience via Tevera)

  • Site Evaluation form (to be completed by student)

Practicum and Internship Application Steps


  • Confirm with your advisor you have met all prerequisites.
  • Visit the P/I Webpage for the current application deadline. All application requirements must be submitted by the deadline.
  • Thoroughly read the Clinical Handbook for additional information.
  • Create a Tevera account. If a Tevera account is needed, please contact the Coordinator of Clinical Experiences for a link to set up your account.
  • To review the list of possible clinical sites using Tevera, visit “Site Placements” and then select “My Sites”.
  • Begin contracting possible sites/site supervisors, scheduling interviews if required. Please reference the “Site Supervisor Requirements” document found on the P/I Webpage.
  • Once a site has been confirmed, it is the student’s responsibility to become familiar with all onboarding requirements of the selected sites. All onboarding must be completed prior to the first day of the semester. It is recommended that students check with the intended site at least two months prior to the start of the semester to determine onboarding requirements. Please note, some school districts may require a State Police or FBI criminal background check, in addition to the required EKU background check.
  • To complete the application on Tevera, select “Site Placements”, “My Site Placements”. Next, select the applicable tab (Practicum, Internship I or Internship II). You will then complete all required steps found under 1. Pre-Application Tasks, 2. Site Details and 3. Supervisor Details. Please reference the Appendices of the Clinical Handbook for step-by-step directions.

  • Applications must be approved by the COU Office. Approximately 3 weeks after the close of the application window, students will be notified if requirements are missing from the application and what additional information is needed. If all requirements are met, the CRN to register for the class will be sent approximately 3 weeks after the close of the application window.

Clinical Mental Health Counseling Students ONLY: LPCA Application (to be completed during the LAST semester of Internship) Complete to apply for licensure as a Licensed Professional Counselor Associate (LPCA).

Application for Licensure as an LPCA
